Telehealth Informed Consent

What to understand and agree to before receiving care via telehealth.

What telehealth is

Telehealth means receiving care from a licensed healthcare provider using secure electronic communications, such as online questionnaires, secure messaging, phone, or video, instead of an in-person office visit. Through VidaFlow Health you complete a secure medical intake and connect with a licensed provider who reviews your information and decides whether treatment is appropriate for you.

This page explains what telehealth involves, its benefits and limits, and what you are agreeing to before you receive care. Please read it carefully. By moving forward with a consultation or intake, you confirm that you have read and understood this consent.

Who provides your care

VidaFlow Health is a technology platform. It is not a medical practice, is not a healthcare provider, and does not practice medicine or provide medical advice. VidaFlow Health connects you with an independent professional medical practice and with independent licensed pharmacies.

Your clinical care is provided by VidaFlow Medical Group, an independent licensed professional medical practice, and its providers (providers@vidaflowhealth.com, 1-800-000-0000, 456 Example Avenue, Anytown, ST 00000). Any medication is dispensed by independent licensed US compounding pharmacies, which are named on our Pharmacies page. These providers and pharmacies are responsible for their own clinical decisions and for your treatment records.

Completing an assessment or consultation does not create a doctor-patient relationship with VidaFlow Health. A provider-patient relationship is formed only with the independent provider, and only if that provider accepts you for care.

Eligibility: 18+ and where services are available

You must be at least 18 years old to use these services. Services are intended for adults only.

Telehealth services are available only in states where the providers are licensed and where the law permits care to be delivered and medication to be prescribed this way. Availability can change, and some treatments may not be offered in your state. If services are not available where you are located, a provider cannot treat you.

Prescribing always follows the laws of the state where you are located at the time of your visit, including that state's telehealth and prescribing rules. You agree to give your accurate current location and to use these services only from a state where they are available to you.

How care begins (no on-site checkout)

VidaFlow Health is a concierge service offered by application. The marketing website has no on-site checkout. Care begins with a free consultation request through our booking quiz. After your consultation, you complete a secure medical intake, and any payment, medical records, and ongoing care take place on the separate provider platform at portal.vidaflowhealth.com.

Submitting a consultation request or intake does not guarantee that you will be accepted for care or prescribed any treatment. Acceptance is always at the provider's discretion.

Benefits of telehealth

Telehealth can make care more convenient and private. Potential benefits include:

  • Access to a licensed provider without traveling to an office or sitting in a waiting room.
  • Discreet, private care and shipping in plain packaging when medication is prescribed.
  • Ongoing secure messaging with your care team between visits.
  • Faster initial review than many traditional in-person scheduling timelines.

Risks and limitations of telehealth

Telehealth is not right for every person or every condition. You should understand the following before you begin:

  • Without an in-person physical exam, a provider works from the information you provide and may have a less complete picture than during an office visit. Giving inaccurate or incomplete information can lead to a wrong decision or harm.
  • A provider may decide telehealth is not appropriate for you and may recommend that you be seen in person.
  • Technology problems, delays, or interruptions can affect or delay your care.
  • Electronic communications, while protected by reasonable safeguards, carry a small inherent risk that information could be accessed by others.
  • As with any medical treatment, there are possible side effects and risks, and outcomes cannot be promised.

Medications and prescriptions

Whether you receive any prescription, and which medication and dose, is decided solely by your licensed provider based on your intake, history, and applicable law. A provider may decline to prescribe, may prescribe something different than you expected, or may stop a treatment at any time if it is no longer appropriate. No medication is guaranteed.

Some of the medications offered through this platform are compounded by licensed US pharmacies. Compounded medications are prepared for an individual patient and are NOT reviewed or approved by the FDA for safety or effectiveness. You should discuss the benefits and risks of any compounded medication with your provider before starting.

Any prescription, including any controlled or otherwise regulated medication, is dispensed only when a provider determines it is clinically appropriate and only as permitted by federal and state law. Your provider, in their sole professional judgment, decides whether such a medication is suitable for you. You will not buy, share, or resell any medication and will use it only as directed.

Results are not guaranteed

Medicine cannot promise a specific result. Individual responses to treatment vary, and the same treatment can work differently for different people. VidaFlow Health and the independent providers and pharmacies do not guarantee any particular outcome, weight change, or health result. Information on this site is for general education and is not a promise of results.

Your responsibilities

For your safety, you agree to:

  • Provide accurate, complete, and current health information, including medications, allergies, and medical conditions.
  • Follow your provider's instructions and the directions that come with any medication.
  • Report side effects or concerns to your care team promptly.
  • Keep your contact and location information up to date.
  • Seek in-person or emergency care when your condition calls for it.

Records and continuity of care

Your medical records are created and kept by the independent provider and pharmacies that care for you, on the provider platform at portal.vidaflowhealth.com. You can message your care team there with questions, refill requests, or changes to your health. Telehealth through this platform is not a substitute for a relationship with a local primary care provider, and we encourage you to keep one and to share your treatment information with them.

In an emergency, call 911

Telehealth and this platform are not for emergencies and are not monitored in real time. If you have a medical emergency, or thoughts of harming yourself or others, do not use this site or wait for a message reply. Call 911 immediately or go to the nearest emergency room. You can also reach the 988 Suicide and Crisis Lifeline by calling or texting 988.

Privacy and electronic communications

By using the service, you consent to receive care, documents, and communications electronically. Health information you share is handled in line with our Privacy Policy and applicable law. Health information held by the independent providers and pharmacies may be protected under laws such as HIPAA, and those parties maintain their own privacy practices that govern your treatment records. VidaFlow Health, as a technology platform, is not itself a HIPAA covered entity.

Your consent, and your right to withdraw it

By requesting a consultation, completing an intake, or otherwise using the service, you confirm that you are at least 18 years old, that you have read and understood this consent, that you have had the chance to ask questions, and that you voluntarily agree to receive care through telehealth.

You may withdraw your consent to telehealth at any time by contacting your care team or care@vidaflowhealth.com. Withdrawing consent stops future telehealth care but does not undo care already provided. Withdrawing consent does not by itself cancel any payment obligations, which are handled under the terms of the provider platform.
